INDUSTRIAL ACCIDENTS


Industrial Accidents can be categorized as incidents that occur within an industrial setting. There are a wide variety of accidents that may occur in this type of environment. Some examples include:

• Machinery failure
• Crane failure
• Pressure vessel failure
• Piping failure

This photograph shows an example of an overhead gantry crane failure. Examination of the crane assembly revealed several tell-tale signs that described the process in which the crane failed. The primary instigator of the catastrophic failure was a fatigued weld. The weld involved had cracked and then completely failed; subsequently, the fractured weld surfaces were abraded to a smooth finish as shown in the image below.
